چکیده

We illustrate the valuable information provided by echocardiography for hemodynamic monitoring and for optimizing ventilatory strategies, during ARDS. Although the transthoracic and the transesophageal routes can be used, we always prefer, in the absence of contraindications, to perform transesophageal echocardiography. ARDS includes numerous affections which brutally damage the interface between the distal airway tract and pulmonary vascular bed. Two factors combine to produce right ventricular systolic overload, the pathologic features of the syndrome per se and mechanical ventilation. Acute cor pulmonale (ACP) reflects the severity of the pulmonary disease, but may also be caused or exacerbated by an aggressive and unsuitable ventilatory strategy. With tidal volume limitation, the incidence of ACP has declined to 25%. Providing that ventilatory management is adapted to right ventricular function, ACP is no longer significantly associated with increased mortality. If not, it is demonstrated that right ventricular dysfunction is actually associated with a poor

Assessment of right ventricular function.

PURPOSE OF REVIEW To emphasize the importance and clinical implications of right ventricular function assessment in many situations encountered by intensivists and to explain in practical terms how to perform such an assessment at the bedside. RECENT FINDINGS Assessment of right ventricular function requires a full understanding of the physiology of the right ventricle. The right ventricle, u...

The assessment of cardiac function in veno-venous extracorporeal membrane oxygenation: the emerging role of bedside echocardiography

The management of patients with acute respiratory distress syndrome (ARDS) has deeply changed in recent years. While the ventilatory strategy was initially focused on the lung, now it also considers pulmonary vascular injury and its effects on the right ventricle and on hemodynamics. This is mainly due to echocardiography, which allows intensivists to evaluate accurately, non invasively and at ...

Rhabdomyolysis and Acute Respiratory Distress Syndrome Secondary to Multiple Wasp Stings

Background: Wasp stings usually cause local reactions such as itching, urticaria, angioedema and anaphylaxis. Life-threatening complications following multiple wasp stings are relatively rare and unexpected. However, rhabdomyolysis and acute respiratory distress syndrome following wasp stings are possible. Case presentation: A middle-aged male worker was stung by a swarm of wasps all over his b...

Validity of Spo2/Fio2 Ratio in Detection of Acute Lung Injury and Acute Respiratory Distress Syndrome

Introduction:  One ofdiagnostic criteria for Acute Lung Injury and Acute Respiratory Distress Syndrome is  pao2/fio2 (PF) ratio 300 for ALI or 200 for ARDS. This criteria requires invasive arterial sampling. Measurement of Spo2/Fio2 (SF) ratio by pulseoximetry may be a reliable non invasive alternative to the PF ratio.   Methods and Materials: In a cross sectional study we enrolled 105 sample o...

[Acute respiratory distress syndrome, mechanical ventilation and right ventricular function].

Mechanical ventilation in acute respiratory distress syndrome (ARDS) implies an increase in alveolar and transpulmonary pressure, giving rise to major alterations in pulmonary circulation and causing right ventricular functional overload that can lead to ventricular failure and thus to acute cor pulmonale. The condition is echocardiographically characterized by dilatation of the right ventricle...
